• Know the Required Supporting Documents*
• Original, signed DC OneApp Supporting Documents Checklist and Agreement.
• One Student Aid Report (SAR), PDF or eight (8) page paper version, as provided by the US Department of Education after the completion of the Free Application for Federal Student Aid (FAFSA). 
• A certified copy of the DC Income Tax Return (D-40), including Schedule "S" with the applicant listed as dependent (if applicable), for the most recent calendar year.  Applicants who were not enrolled in school during the most recent calendar year may be required to submit certified D-40s, or other domicile information for previous years.  Applicants may also submit an original, signed Office of Tax & Revenue (OTR) Authorization Form, which will only be available from April 15 through August 29, 2008.  The OTR Authorization Form can be found in the DC OneApp 'Part 3: Sign & Mail Forms & Documents'.

A twelve (12) month income/benefit history statement (for the previous year) from either of the following sources: Temporary Assistance for Needy Families (TANF), Social Security Income (SSI), Social Security Disability Income (SSDI), Retirement Income, Child Support, Worker's Compensation, Veterans Administration, or Unemployment.

• One current utility bill, phone bill (land line, not cell phone), bank statement, pay stub or mortgage statement, no older than forty-five (45) days from the date of the DC OneApp submission that reflects the name and address of either the applicant or their parent/legal guardian.
• New applicants must provide proof of high school completion (i.e. high school diploma, General Equivalency Diploma (GED) certificate, college acceptance letter or college transcript).  Current high school students may provide a signed Satisfactory Academic Progress Toward Graduation (SAP) Form with original signatures.  The SAP Form can be found in the DC OneApp "Part 3: Sign & Mail Forms & Documents". 
• Affirmation and Legal Disclaimer Statement with original signature from the applicant and the parent/legal guardian (if applicant is a dependent).  The Affirmation and Legal Disclaimer Statement can be found in the DC OneApp "Part 3: Sign & Mail Forms & Documents".
